MA Motor Vehicle Operator's Crash Report


	If you have been involved in a motor vehicle accident, you must fill out a Massachusetts Motor Vehicle Crash Operator's Report. You can download the form here: www.mass.gov/rmv/forms/accident.htm or pick them up at any police station or Mass RMV. You will need 3 copies of this form: 1 for the local police department where the ACCIDENT OCCURED, 1 for the Mass RMV, 1 for your insurance company. You have 5 business days after the accident to complete and file these forms. If you need a copy of an accident report (officer's report) you may call the Ware Police Department - Records Division Monday-Friday 8am - 4pm, excluding holidays, 413.967.3571.
