The Ware Board of Health would like to remind all local businesses obtaining Board of Health issued license/permit of the expiration date of December 31, 2009.
It is the establishment owners and or managers responsibility to obtain this license/permit annually. Any establishment that has not picked up and paid for their license/permit by December 31, 2009 will be operating without a valid license or permit.
In order to renew permits and licenses, an updated application form must be submitted. Also according to MGL, Chapter 152, section 25A, all licensing authorities in a community must ensure that prior to the issuance of any license or permit one of the following must be completed on an Affidavit:
1. A completed Affidavit giving worker's compensation coverage held.
2. A sworn Affidavit by the individual before the issuance of any license or permit that they are not required having worker's compensation insurance coverage.
