REPORT IMMEDIATELY
The following diseases must be reported immediately to the Ware Board of Health Office (if the case resides in Ware)  by telephone  413.967.9615  or by fax 413.967.9646 and to the Massachusetts Department of Public Health at 617.983.6800(weekdays) After business hours call  MDPH 617.522.3700 (24 hours/7 days)

» Haemophilus Influenzae
» Hepatitis A in a Foodhandler
» Measles
» Meningococcal Infection
» Poliomyelitis
» Rabies
» Rubella, Congenital & Non-Congenital

» Any cluster/outbreak of illness
» Enteric illnes in a foodhandler>
» Any cluster of work-related conditions, regardless of whether or not they are on the reportable list.
